High Peaks Hospice & Palliative Care recently announced the formation of a new Community Outreach Team. The Community Outreach Coordinator, Development Coordinator and Professional Liaison will work together to provide information, education and collaborative opportunities to the communities of Warren County.<\/p>\n
The Community Outreach Team is responsible for enhancing and maintaining the quality of the relationship between High Peaks Hospice & Palliative Care and the community at large. The team cultivates program awareness, referrals and relationships using a multi-faceted approach including education, advocacy, and enhanced community presence.<\/p>\n
The team consists of; Fran Feathers, Community Outreach Coordinator, Sunday Conine, Development Coordinator and Sue Gillingham, LPN, Professional Liaison. Each will be responsible for specific demographics within the community to assure that all areas of the community have access to information and service.
