Adirondack Health Institute (AHI) is partnering with CVPH Medical Center, Hudson Headwaters Health Network (HHHN) and Washington County Public Health to implement and expand diabetes prevention efforts in six Adirondack-region counties.<\/p>\n
The Centers for Disease Control and Prevention National Diabetes Prevention Recognition Program launched earlier this year in Clinton, Essex, Franklin, Hamilton, Warren and Washington counties. The state Health Foundation (NYSHealth) awarded AHI a grant to support the initiative.<\/p>\n
“Adirondack Health Institute is both the facilitator and lead organization for this grant to build the infrastructure to develop a network of certified lifestyle coaches to deliver the National Diabetes Prevention Recognition Program,” said Bob Cawley, director of medical home initiatives, Adirondack Health Institute.
\n“We are collaborating with existing partners in developing a referral
\nbase from providers and employers who have established workplace
\nwellness programs to train the lifestyle coaches and implement this
\nvital regional health care program.”<\/p>\n
The initiative covers a largely rural population of approximately 300,000 residents, approximately 13 percent of whom live under the federal poverty level. The funding will be used to start up a regional diabetes prevention program targeted at those with pre-diabetes or are at risk for diabetes, while more permanent solutions are being developed.<\/p>\n
“Although there are limited local health statistics available on patient population for both pre-diabetes and diabetes in our six-county region, we have higher obesity, smoking, and high blood pressure rates and our patients may also likely be unaware of their risk for diabetes,” said Cawley.<\/p>\n
The following employers have made initial commitments to implementing the Diabetes Prevention Recognition Program to complement their existing wellness programs:<\/p>\n
\u2022 Adirondack Health, the largest private employer inside the Adirondack Park with 900 employees located in five practice sites, has a very active wellness committee.<\/p>\n
\u2022 CVPH Medical Center has approximately 2,000 employees and an established wellness program.<\/p>\n
\u2022 Hudson Headwaters Health Network is a large federally-qualified health center system with 16 locations and 600 employees.<\/p>\n
\u2022 Washington County Public Health has approximately 800 employees with an established wellness program.<\/p>\n
It is expected that Adirondack Health Institute and their partner organizations will be providing outreach to regional employers and providers, making them aware of the program, as well as promoting the effectiveness of the program in the workplace.<\/p>\n
“Once our partner organization’s lifestyle coaches are trained, we will encourage additional regional organizations to include the National Diabetes Prevention Recognition Program as part of their mission to continue this program,” said Cawley.<\/p>\n
The Adirondack Health Institute, a nonprofit organization licensed under the state Public Health Law as an Article 28 Central Services Facility, expands regional collaboration among health care and social service providers serving the Adirondack Region of New York state.<\/p>\n
