Adirondack Health Institute (AHI) and
\nits Performing Provider System (PPS) partners
\nhave received $891,000 in grant funding
\nfrom the state Department of Health
\nfor the Delivery System Reform Incentive
\nPayment (DSRIP) Program’s Project Design
\nGrant.<\/p>\n
Funding will be used to increase the level
\nof health care service integration necessary
\nto better manage population health and
\nembrace payment reform in nine Upstate
\nNew York counties. AHI is one of 42 grant
\nawardees in the state.<\/p>\n
The project will be implemented by partners
\nin Clinton, Essex, Franklin, Fulton,
\nHamilton, Saratoga, St. Lawrence, Warren
\nand Washington counties.
\n“The North Country Health Systems
\nRedesign Commission issued a report in April of this year describing our region’s
\nhealth care system as ‘highly fragmented
\nand centered on inpatient beds.’ To address
\nthis issue, AHI convened a Project Advisory
\nCommittee (PAC) which agreed early on
\nthat creating an integrated delivery system
\nwould be the core focus of our project
\nproposal,” said Cathy Homkey, CEO of AHI.<\/p>\n
“We’re extremely pleased to receive this
\ngrant and excited to work with our numerous
\ncollaborators to develop a comprehensive
\nand successful health care integration
\nplan. The award reflects the confidence that
\nthe state has in the ability of our region’s
\nhospitals, primary care, and other providers
\nto work collaboratively.”<\/p>\n
Homkey said AHI continues to encourage
\ninterested organizations to join the effort.
\n“The PAC is growing by bringing aboard partners to represent the workforce component
\nof this initiative, which will be coordinated
\nby the Hudson Mohawk Area Health
\nEducation Center, a partner of AHI,” said
\nHomkey. “As we move forward, we hope to
\nbring other key stakeholders aboard.”<\/p>\n
Colleen Florio, director of care management
\ninitiatives and evaluation at AHI said
\nin order for the system to succeed, “it needs
\nto provide access to high-quality primary
\ncare, behavioral health services, and substance
\nabuse treatment, and we need to better coordinate and manage care across
\nthese settings. We also need to focus on
\nwhen and where care is delivered to address
\nthe over-reliance on inpatient beds noted by
\nthe commission.”<\/p>\n
“As such, our project, among other areas
\nof focus, will concentrate on integrating
\ncare and providing more community-based
\noptions and supports. We look forward to
\nbuilding healthy communities and helping
\nour partners succeed under payment
