The Adirondack Health Institute (AHI)
\nMedical Home in Glens Falls is one of five
\nMulti-payer Advanced Primary Care Practices
\n(MAPCP) demonstrations nationwide
\nto receive an extension by Centers for Medicare
\n& Medicaid Services (CMS).<\/p>\n
The extension runs through Dec. 31,
\n2016.<\/p>\n
Launched in 2011, the Adirondack
\nMedical Home was a five-year pilot study.
\nThe health care goals included improving
\nquality, ensuring access and containing
\ncosts. The emphasis continues to be on
\npreventive care, enhanced management of
\nchronic conditions, and the assurance of
\na close relationship between patients and
\ntheir primary care providers.
\nJason Helgerson, New York state medicaid
\ndirector, recently praised Adirondack
\nMedical Home’s national success at a statewide
\nmeeting.
\n“They (AHI) have been actively involved
\nin one of the nation’s most successful
\npatient medical-home pilots that brought
\ntogether a whole array of providers who
\nhave actively participated in the Medicaid
\nprogram,” he said.<\/p>\n
Bob Cawley, AHI’s Medical Home initiatives
\ndirector, presented its successful
\nimplementation strategies to multi-stakeholder
\nrepresentatives of the Comprehensive
\nPrimary Care (CPC), a national medical
\nhome initiative.<\/p>\n
“We wanted to invite AHI because of
\ntheir recognized national success status,”
\nsaid Lisa Dulsky Watkins, director, Milbank
\nMemorial Fund Multi-State Collaborative,
\nthe convener of CPC and MAPCP forums.
\n“They serve as exemplars for our program and we felt it was important to benefit from
\nlessons learned.”<\/p>\n
The Adirondack Medical Home is a collaborative
\neffort by health care providers
\nand public and private insurers to transform
\nthe health care delivery system in a rural,
\nupstate New York region. Forty-one practices
\nin the Adirondack region (Clinton,
\nEssex, Franklin, Hamilton, Warren, and
\nWashington counties) currently participate
\nin the Adirondack Medical Home initiative.<\/p>\n
The traditional health care model
\n(known as “fee for service”) pays physicians
\nfor the volume of claims submitted, instead
\nof the results of their care. Important
\npreventive care is often not reimbursed
\nor underpaid, so patients end up getting
\ntreated only when a disease or complaint
\nbecomes severe. There are also no incentives
\nfor coordination of care and managing
\nchronic conditions.<\/p>\n
“The Adirondack Medical Home shifted
\nthe focus to preventive care and health
\nmaintenance,” said Cawley. “The pilot
\ncreated funding incentives that rewarded
\nkeeping people as healthy as possible rather
\nthan paying for procedures without regard
\nto effectiveness. This approach allows
\npatients to stay healthier and doctors to
\nconcentrate more on quality of care rather
\nthan volume of care.”<\/p>\n
On the patient side, officials said increased
\ncontact with the primary care
\npractitioner enables earlier diagnosis and
\nprevention of problems. Early diagnoses
\ngenerally translate into better health and
\nlower costs.<\/p>\n
