Adirondack Health Institute (AHI) in Glens
\nFalls and its Performing Provider System (PPS)
\npartners have the opportunity to earn up to
\n$186.7 million in performance-based payments
\nfrom the state Department of Health (DOH)
\nunder the Delivery System Reform Incentive
\nPayment (DSRIP) Program’s Project Plan.<\/p>\n
State officials said funding will be used to increase
\nthe level of health care service integration
\nnecessary to better manage population health
\nand embrace payment reform in nine Upstate
\nNew York counties including Warren, Washington
\nand Saratoga.<\/p>\n
The AHI PPS is one of 25 awardees in the state.
\n“Our region has been recognized for its strong
\npartnerships and we’re pleased by the state’s
\ninvestment in healthcare system transformation
\nand support of our regional initiatives,”
\nsaid Cathy Homkey, CEO of AHI.
\n“The DOH has
\nannounced the maximum award our region can
\nearn. This number is based on our ability to hit
\neach milestone.”<\/p>\n
The AHI PPS spans nearly 11,000 square miles,
\naffecting approximately 144,000 attributed lives.
\nThe project will be implemented by partners
\nin Clinton, Essex, Franklin, Fulton, Hamilton,
\nSaratoga, St. Lawrence, Warren, and Washington
\ncounties.<\/p>\n
AHI established four Regional Health Innovation
\nTeams led by experts in health care
\ntransformation from primary care, acute care,
\nand behavioral health settings, and each team
\nincludes representation from the full care
\ncontinuum.<\/p>\n
“The geography requires strong localized
\nplanning units to develop plans and improve
\nperformance,” said Homkey. “We believe this
\nstructure will be essential to implementing
\nrapid-cycle change processes.”<\/p>\n
The teams solicited partners, set goals, and
\ndeveloped projects which formed the basis of an
\napplication submitted to the DOH in December
\n2014.<\/p>\n
“DSRIP provides our region with the opportunity
\nto redesign the system of care to improve the
\nhealth of the nearly 700,000 residents of our nine county service area,” said Dr. Colleen Florio AHI’s
\nvice president of health system transformation.
\n“The recent funding is focused on Medicaid, but
\nthe entire region benefits from dollars provided
\nto support population health.”<\/p>\n
Payments to providers will be based on
\nimprovements in patient outcomes, utilizing
\nincentives to get providers to work together to
\noffer preventive, rather than reactive care; integrate
\nbehavioral- and primary care services; offer
\nmore home and community-based services; and
\naddress the social determinants of health, such
\nas nutrition, housing and employment.<\/p>\n
Each region has selected projects to develop.
\nIn the nine-county AHI region, the team focused
\non 11 projects.
\n“Four of the 11 projects are behavioral health
\nfocused,” said Florio. “A majority of emergencydepartment
\nvisits are related to, or complicated
\nby, behavioral health conditions, thus, the Incentive
\nPayment program places a large emphasis
\non better serving individuals with behavioral
\nhealth care needs.”<\/p>\n
New York state is re-investing nearly $6.5
\nbillion in health-care delivery system redesign
\nstatewide. Officials said the goals are to reduce
\navoidable hospital use by 25 percent over five
\nyears and ensure that New Yorkers achieve
\nhigh-quality care, better health outcomes and
\nlower cost. AHI was awarded a one-year project
\ndesign planning grant ($891,000) in July 2014,
\nand received an additional $1.2 million in planning
\ndollars in early 2015.<\/p>\n
The funds were earmarked to develop the
\nPerforming Provider System’s governance and
\ninfrastructure, and to plan 11 projects in concert
\nwith partner organizations.<\/p>\n
