With the goal of promoting the triple aim
\nof improved population health, better patient
\nexperiences and quality of care, and reduced
\nhealth care costs, AHI in Glens Falls (Adirondack
\nHealth Institute) recently convened a group
\nof more than 50 diverse stakeholders for the
\nkick-off of the Population Health Improvement
\nProgram (PHIP).<\/p>\n
PHIP is a state Department of Health program
\nthat supports the state’s Prevention Agenda. The
\nkick-off was the first step in creating a six-county
\nNorth Country Population Health Collaborative
\n(NCPHIC).<\/p>\n
“We are very excited about this initiative and
\nthe large turnout of invested partners who came
\ntogether today,” said Cathy Homkey, AHI CEO.
\n“This initiative will have far-ranging impact,
\nnot only on promoting population health, but
\nalso in its support and advancement of other
\nrelated population health and health system
\nimprovement initiatives.”<\/p>\n
In addition to introducing the program, the
\nevent involved looking at various factors that
\nhave an impact on population health. Anne-Marie
\nFitzGerald, AHI community health services
\nevaluation manager, introduced an example of
\nhow a single socio-economic trend – the difficulty
\nof finding affordable housing – could have serious
\nimplications on a person’s health.<\/p>\n
“If you can’t afford your home, it could lead
\nto poor nutrition as dollars to spend on healthy
\nfoods are scant. That could lead to stress and
\nmental health issues as managing bills becomes
\noverwhelming. It could also lead to higher
\nutilization of emergency room visits as preventive
\nmedicine,” said FitzGerald. “So, one factor can
\nhave a devastating trickle-down effect on a
\nperson’s well-being.”<\/p>\n
Lottie Jameson, AHI vice president of regional
\nplanning and development, said the meeting
\nprovided an opportunity to get various partners
\ntalking about how the sharing of data from other
\nsectors could help everyone’s programs and
\nservices be more effective.<\/p>\n
“Think about the meaningful change we could collectively generate if we work together to make
\ninformed community-based and data-driven
\ndecisions and policies,” said Jameson. “With all
\nthe data out there, the question is how best to
\norganize it in one place and bring it to life through
\nsharing and stories. One comprehensive, regional
\nassessment that would, in effect, combine all
\nthe current assessments conducted by various
\norganizations would be a huge accomplishment
\nand something this group will explore.”<\/p>\n
Suzanne Lavigne, director of Franklin County
\nCommunity Services, said there is value in
\nsharing ideas and data with various partners from
\nother sectors. That can help everyone’s programs
\nand services be more effective.<\/p>\n
“Our counties have a tremendous opportunity
\nto chart a new course in how we support
\npopulation health and wellness. Agency
\ncollaboration will be critical in order to establish
\npriorities and achieve successful outcomes that
\nwill impact individuals, families and generations
\nto come,” she said.<\/p>\n
Megan Murphy, AHI director, community
\nhealth services said the next steps include
\nconvening quarterly meetings to determine
\nstakeholders’ needs and how PHIP’s resources
\ncan best support their work.<\/p>\n
“Another exciting development will be the
\ncreation of a HealthyADK regional website that
\nwill have user-friendly access to community
\nhealth data, links to evidence-based practice
\nstrategies and information on initiatives in our
\nregion,” she said.<\/p>\n
The North Country PHIP, one of 11 PHIPs
\nin the state, covers Clinton, Essex, Franklin,
\nHamilton, Warren, and Washington counties.
