{"id":18083,"date":"2015-10-15T17:17:18","date_gmt":"2015-10-15T21:17:18","guid":{"rendered":"https:\/\/www.glensfalls.com\/glensfallsbusinessjournal\/2015\/10\/ahi-brings-stakeholders-together-to-discuss-integrating-health-systems-in-north-country.html"},"modified":"2015-10-15T17:17:18","modified_gmt":"2015-10-15T21:17:18","slug":"ahi-brings-stakeholders-together-to-discuss-integrating-health-systems-in-north-country","status":"publish","type":"post","link":"https:\/\/www.glensfalls.com\/glensfallsbusinessjournal\/2015\/10\/ahi-brings-stakeholders-together-to-discuss-integrating-health-systems-in-north-country\/","title":{"rendered":"AHI Brings Stakeholders Together To Discuss Integrating Health Systems In North Country"},"content":{"rendered":"<div class=\"img-left\">\n<div>\n<img loading=\"lazy\" decoding=\"async\" alt=\"ahi forum hc.jpg\" src=\"https:\/\/www.glensfalls.com\/glensfallsbusinessjournal\/wp-content\/uploads\/sites\/109\/2015\/10\/ahi20forum20hc.jpg\" class=\"aligncenter\" height=\"229\" width=\"300\">\n<\/div>\n<div class=\"img-caption\">A group of panelists, including Annette Parisi of AHI and Brian McDermott of Glens Falls Hospital shared insights for transforming the region&#8217;s health care system\n<\/div>\n<\/div>\n<p>Regional health care professionals, community<br \/>\nleaders, and a panel of local experts, explored<br \/>\nthe challenges and approaches associated with<br \/>\ntransforming the Adirondack\/North Country region<br \/>\ninto a high-performing integrated health<br \/>\nsystem at Adirondack Health Institute&#8217;s recently held<br \/>\nannual summit.<\/p>\n<p>More than 200 individuals attended<br \/>\nIntegrating Health Care in the Adirondacks,<br \/>\nSept. 24 in Lake Placid.<br \/>\nCreating an integrated health care delivery<br \/>\nsystem centered on evidence-based medicine<br \/>\nand population health management across the<br \/>\nAdirondack region was the focus of the event.<\/p>\n<p>A diverse and dynamic group of panelists shared<br \/>\ninsights, best practices, and recommendations<br \/>\nfor transforming the region&#8217;s health care system.<br \/>\nThey were:<br \/>\nKaren Ashline, Adirondacks ACO; Annette<br \/>\nParisi, AHI; Dr. Brian McDermott, Glens Falls<br \/>\nHospital; Scott Momrow, Hixny; and Peter Trout,<br \/>\nClinton County Mental Health &amp; Addiction<br \/>\nServices, Moderator Hope Plavin, Innovation<br \/>\nCenter, state Department of Health.<\/p>\n<p>Health care providers, administrators,<br \/>\nbusiness, government and association<br \/>\nrepresentatives, and other key stakeholders<br \/>\nfrom nine North Country counties &#8212; Clinton,<br \/>\nEssex, Franklin, Fulton, Hamilton, Saratoga, St.<br \/>\nLawrence, Warren and Washington &#8211; attended<br \/>\nthe event.<\/p>\n<p>Panelists emphasized a more patient-centered<br \/>\napproach.<br \/>\n&#8220;At the end of the day it&#8217;s about the patient.<br \/>\nWe try to measure the system, but should focus<br \/>\non the needs and successes of the patient,&#8221; said<br \/>\nMcDermott.<\/p>\n<p>Ashline added that caregivers shouldn&#8217;t solely<br \/>\nfocus on individuals who enter the health care<br \/>\nsystem.<br \/>\n&#8220;We can track the patients who come to the<br \/>\nED (emergency department). It&#8217;s the individuals<br \/>\nthat we don&#8217;t know about who fall through<br \/>\nthe cracks,&#8221; she said. &#8220;We need to identify and<br \/>\nsupport those individuals before they become<br \/>\nchronically ill.&#8221;<\/p>\n<p>&#8220;The health care landscape is ever-changing<br \/>\nand new challenges for providing quality, easily accessible<br \/>\nand affordable care arise every day.<\/p>\n<p>The North Country is no exception and faces<br \/>\nperhaps even more difficult care provision<br \/>\nchallenges because it is so vast and remote,&#8221; said<br \/>\nCathy Homkey, CEO of AHI. &#8220;It&#8217;s important AHI<br \/>\ncoordinates forums such as our summit, because<br \/>\nit allows those involved in the process to hear<br \/>\nfrom experts in order to collectively formulate<br \/>\na vision and game plan for insuring a healthy<br \/>\nfuture for our region.&#8221;<\/p>\n<p>Other topics explored at the event included:<br \/>\nDr. Jacob Reider, chief strategy officer, Kyron,<br \/>\nInc., an expert in health information technology<br \/>\npolicy, innovation, and strategy, discussed the<br \/>\nrural impact of HIT in a morning keynote<br \/>\npresentation.<\/p>\n<p>Gloria Kupferman, vice president, DataGen,<br \/>\ndiscussed patient-centered analytics to support<br \/>\ncare redesign and new payment models in an<br \/>\nafternoon keynote presentation.<\/p>\n<p>The summit is one of the many community<br \/>\nservices provided by AHI (Adirondack Health<br \/>\nInstitute).
