Fort Hudson Home Care, an affiliate of Fort Hudson Health System, received approval from the state Department of Health to expand its current service area to include Clinton County.<\/p>\n
Fort Hudson currently operates this program in Saratoga, Warren, Washington, Hamilton and Essex counties, and provides over 200,000 hours of service annually.<\/p>\n
Fort Hudson Home Care provides a range of services to individuals requiring support and assistance in the community. In addition to the personal care services, Fort Hudson provides service coordination to over 100 individuals living in the community through the Nursing Home Transition and Diversion Medicaid waiver program and Traumatic Brain Injury waiver program.<\/p>\n
According to Beth Bruno, RN, director of home and community services at Fort Hudson, “The expansion into Clinton County reflects the growing demand for our services, primarily the provision of case management for Medicaid enrollees. Our depth of experience in case management is another value which we will now bring to Clinton County.”
\nFort Hudson provides case management services under contract with managed care plans for a growing number of individuals as Medicaid enrollees are transitioned into Managed Medicaid plans as part of a statewide initiative to improve outcomes and control health care cost.<\/p>\n
